Key Responsibilities\n

  Conduct comprehensive assessments of patients' psychosocial needs to identify appropriate treatment pathways, including immediate and long-term counseling, crisis intervention, and referrals as needed.

  Assist patients in navigating entitlement programs and completing necessary forms for supplemental insurance and other benefits, with the aim of reducing dependence on emergency assistance.

  Facilitate access to prescription medications by sourcing cost assistance through pharmaceutical assistance programs and coordinating with community resources.

  Act as a patient advocate when interfacing with community agencies, ensuring clients receive the comprehensive support they require.

  Maintain accurate and current patient records, generating reports and documentation as necessary to fulfill both clinical and regulatory requirements.

  Coordinate transportation assistance for patients requiring help in getting to essential appointments or therapy sessions.

  Participate in regular clinical meetings and case conferences to discuss patient progress and collaborate on the development of individualized treatment plans.

Patient Population / Client Focus\n

Qualifications\n
