Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate clients’ mental health needs and determine appropriate interventions.

• Implement and facilitate individual, group, and family therapy sessions, utilizing evidence-based practices to help clients navigate their experiences.

• Coordinate with an interdisciplinary treatment team to develop and adjust personalized care plans that address the unique needs of each client.

• Manage cases for assigned patients, providing ongoing support and follow-up to monitor progress and ensure effective outcomes.

• Research and advocate for community resources, such as healthcare services, housing support, and financial assistance programs, to alleviate barriers faced by clients and enhance their stability and quality of life.

• Respond promptly to crisis situations, including child welfare concerns and mental health emergencies, employing appropriate intervention strategies.

• Facilitate referrals to other agencies and professionals as needed, ensuring that clients receive comprehensive care.

• Maintain accurate and timely documentation of patient interactions, therapy sessions, and treatment plans in compliance with regulatory requirements.

Patient Population / Client Focus\n

The role targets a diverse patient population, including children, families, and individuals facing a wide array of challenges such as mental health disorders, trauma, relationship issues, and life transitions. We particularly seek someone with experience working with Veterans, as understanding their unique experiences and barriers is critical in our efforts to provide effective care.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is required.

• A valid and active Licensed Clinical Social Worker (LCSW) license is mandatory.

• At least 2 years of professional experience in a behavioral health setting, demonstrating a solid understanding of therapy methodologies.

• Proficient in conducting psychosocial history reviews and case management evaluations.

• Detail-oriented with exceptional skills in interpersonal communication and crisis intervention.
