Key Responsibilities\n

• Act as a primary liaison between families and outpatient providers, ensuring clear and supportive communication that optimizes patient care and engagement.

• Engage with insurance companies to manage pre-certifications and utilization management processes, facilitating seamless access to necessary services for clients.

• Contribute to comprehensive discharge planning and aftercare strategies that enhance patient transitions and promote ongoing health and wellness.

• Provide therapeutic meal support, empowering clients to build a healthier relationship with food while fostering group discussions around nutrition and self-care.

Patient Population / Client Focus\n

This role specifically focuses on older adults receiving hospice care, often facing complex emotional and psychological challenges. Qualifications\n

Compensation and Benefits\n
