Key Responsibilities\n

• Serve as a key liaison between clients, families, and outpatient providers, ensuring comprehensive understanding and coordination of care plans.

• Engage with insurance companies for pre-certification and utilization management, advocating for clients to access necessary resources effectively.

• Participate actively in discharge and aftercare planning by evaluating clients' progress and facilitating smooth transitions into continued care.

• Provide therapeutic meal support, helping clients navigate their relationships with food and modeling healthy eating behaviors.
\n
• Deliver empathetic and affirming care tailored to the unique circumstances of each client, ensuring sensitivity to their intersecting identities.

Patient Population / Client Focus\n

In this role, you will work primarily with adults undergoing detoxification and recovery from substance use disorders, as well as addressing accompanying mental health challenges. Clients vary widely in their backgrounds, presenting a range of life experiences and cultural contexts that you will have the opportunity to engage with thoughtfully and respectfully.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• A valid LCSW license in the state of New Jersey is required.

Compensation and Benefits\n
