Key Responsibilities\n

• Acting as a liaison between families and outpatient providers to facilitate communication and support ongoing patient care.

• Engaging with insurance companies to navigate pre-certification and utilization management processes, ensuring clients receive necessary services.

• Participating actively in the discharge and aftercare planning, collaborating with other healthcare professionals to create comprehensive post-discharge plans.

• Providing therapeutic meal support, while modeling a healthy relationship with food in the context of mental health and wellness.

• Delivering empathetic, affirming care tailored to each client’s distinctive needs and intersecting identities, thereby strengthening their sense of safety and belonging.

Patient Population / Client Focus\n

This role primarily serves older adults within a hospice center environment. As a Clinical Social Worker, you will provide critical support to clients experiencing end-of-life challenges, addressing the psychological, social, and emotional aspects of care. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
