Key Responsibilities\n

• Conduct thorough and ongoing assessments of clients to identify their specific needs and formulate individualized treatment plans.

• Maintain transparent communication with your supervisors, collaborating closely with direct care staff, family members, and guardians to monitor each client’s progress and adjust strategies as necessary.

• Generate and distribute a tentative schedule for appointments, home visits, and observational sessions to streamline your workflow and ensure consistency in client care.

• Coordinate and execute home visits, including in-home observations, chart reviews, and staff interviews to gather comprehensive insights about a client's situation.

• Oversee the implementation of program plans, ensuring that all interventions are carried out effectively and adapting strategies based on client feedback and outcomes.

• Participate actively in Interdisciplinary Team Meetings, contributing your expertise to foster collaborative approaches to our clients' challenges.

• Complete robust monthly and quarterly progress notes to document significant developments and recommendations for future sessions.

• Develop and facilitate training sessions for team members to ensure they are well-versed in program implementation and effective documentation practices.

• Conduct follow-up interviews and additional home visits to provide ongoing support and adjust treatment plans as necessary, ensuring the best possible outcomes for clients.

• Act as a liaison within the mental health community and with the public, enhancing our outreach efforts and supporting developmental goals.

• Contribute to the creation of training materials and procedural documentation aimed at enhancing the effectiveness of our programs.

Qualifications\n
