Key Responsibilities\n

• Conduct thorough assessments of new and current patients to identify their unique needs and develop appropriate treatment plans tailored to each individual.

• Facilitate communication among supervisors, direct care staff, family members, and other stakeholders regarding patient progress and therapeutic outcomes.

• Provide supervisors with a structured schedule for upcoming appointments, home visits, and therapeutic observations to ensure continuity of care.

• Coordinate in-home visits that include comprehensive observations, staff interviews, and data collection to evaluate treatment effectiveness and areas for improvement.
\n
• Ensure all program plans are executed effectively, making necessary adjustments as required to respond to patient feedback and progress.
\n
• Participate in regular Interdisciplinary Team Meetings and relevant discussions to align treatment strategies and share insights.
\n
• Document monthly and quarterly progress notes that reflect behavioral changes and areas needing attention, supporting both clinical and administrative processes.

• Facilitate training sessions for staff to improve competencies related to program plan implementation and documentation standards.
\n
• Conduct follow-up interactions, maintaining regular home visits when needed, to reinforce the support network around our clients.

• Serve as a key liaison between the clinic and the broader mental health community, fostering supportive relationships that enhance service delivery.

• Contribute to the development of training materials aimed at enhancing service offerings and implementing new programs for patient care enhancements.

Patient Population / Client Focus\n

The Licensed Professional Counselor will work primarily with adults and youth facing various mental health challenges, including but not limited to anxiety disorders, depression, trauma, substance misuse, and issues related to family dynamics. Qualifications\n
