Key Responsibilities\n

• Conduct ongoing, thorough assessments to understand and address the unique needs of each patient served, ensuring tailored interventions and support.

• Facilitate clear and effective communication among supervisors, direct care staff, families, guardians, and other stakeholders regarding clients’ progress and care plans.
\n
• Design and manage a tentative schedule for patient appointments, home visits, and observations to monitor therapeutic advancement and compliance with service delivery.
\n
• Coordinate and conduct home visits that incorporate in-home observations, chart reviews, and staff interviews, thereby directly engaging with the patient's environment.
\n
• Ensure that program plans are implemented effectively, supporting clients in achieving their personal goals and objectives in collaboration with interdisciplinary teams.
\n
• Participate actively in interdisciplinary team meetings to contribute insights and strategize client care while documenting monthly and quarterly progress notes in a timely manner.
\n
• Organize training sessions to certify staff in the effective implementation of treatment plans and relevant documentation processes, promoting consistency and quality across our services.
\n
• Provide follow-up interviews, home visits, and continuous observations to maintain an ongoing supportive presence for clients, ensuring they receive necessary assistance as their needs evolve.

• Maintain comprehensive and accurate records that are readily accessible to authorized parties, ensuring compliance with privacy and ethical guidelines.

• Act as a liaison with public and mental health communities to promote developmental goals and foster arrangements for comprehensive patient care.

• Assist in creating and refining training materials that enhance employee competency and program quality, directly impacting our clients’ outcomes.

Patient Population / Client Focus\n

In this role, you will work with various populations, including both adults and youth, addressing a spectrum of challenges such as family issues, relationship dynamics, substance use, and trauma-affected individuals. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Must hold an active and unrestricted legal license as an LPC in the state of Oregon.

• A minimum of two years of counseling experience in a clinical setting is essential, demonstrating your capacity to manage various cases.

• Specialization in family therapy, couples therapy, substance misuse, or trauma counseling is highly preferred, allowing you to tailor your approach according to client needs.

Compensation and Benefits\n

The annual salary for this role ranges from $90,000 to $100,000, commensurate with experience and caseload capacity.
