Key Responsibilities\n

• Act as a liaison between families and outpatient providers, ensuring smooth communication and coordination of care plans.

• Engage with insurance companies to facilitate pre-certification processes and manage utilization effectively.

• Participate in discharge planning and aftercare strategies, ensuring that patients transition smoothly from hospice care.

• Provide therapeutic meal support, modeling a healthy relationship with food and fostering positive eating habits among clients.

• Deliver empathetic, culturally competent care tailored to meet the individual needs of clients based on their intersecting identities.

• Utilize electronic health record systems to document assessments, treatment plans, and progress notes comprehensively.

Patient Population / Client Focus\n

You will be working primarily with older adults who are facing serious health challenges, including terminal illnesses. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
