This position is remote, allowing you the flexibility to work from the comfort of your home while providing vital services to patients in a hospice center. You will collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, and psychologists, to ensure a holistic approach to care. • Conduct extensive psychosocial assessments to evaluate clients' mental health concerns, strengths, and available resources during their hospice journey.

• Offer individual, family, and group therapy utilizing evidence-based treatment modalities, including cognitive-behavioral therapy (CBT) and solution-focused therapy.

• Develop and implement personalized treatment plans that outline measurable goals and specific interventions related to end-of-life issues.

• Collaborate with a diverse team of healthcare professionals to deliver integrated and coordinated care customized for each patient's needs.

• Provide crisis intervention, perform risk assessments, and create safety plans as situations arise.

• Maintain accurate clinical documentation according to ethical and legal standards, ensuring compliance with state regulations.

• Facilitate therapeutic group sessions, psychoeducation programs, and support groups for both clients and their families.

• Engage in ongoing assessment and adaptation of treatment strategies based on clinical progress and patient feedback.

In this role, you will primarily serve clients in hospice care, who may be experiencing complex emotional challenges related to terminal illness, grief, and loss. Our patient population encompasses a diverse demographic, requiring a culturally competent approach to therapy.

• Master's degree in Social Work from an accredited institution, with a focus on clinical practice.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Tennessee.

• A minimum of two years of hands-on experience in a mental health or outpatient care setting, preferably with experience in hospice or palliative care.

• In-depth knowledge of evidence-based therapeutic modalities and treatment approaches relevant to hospice care.

• Exceptional interpersonal, communication, and assessment skills that foster a trusting therapeutic relationship with patients and their families.

The position offers a competitive salary range of USD 80,000 to 100,000 per year, commensurate with experience and qualifications. In addition, a sign-on bonus is available for qualified candidates.
