Although this is a remote position, your contributions will be directed toward an in-person hospice center that provides end-of-life care. Your role will involve staying engaged through virtual meetings and discussions with multidisciplinary teams to ensure a well-coordinated approach to patient care.

Key Responsibilities\n

• Serve as a primary clinician for patients presenting symptoms of major mental illness and/or severe emotional disorders, delivering individualized therapy sessions via telehealth and, when necessary, in-person visits.

• Conduct initial assessments using evidence-based practices, track client progress, and adapt treatment plans accordingly to meet the evolving needs of patients.

• Create and implement integrated care plans while facilitating communication among clients, their families, and healthcare providers to ensure cohesive treatment pathways.

• Assist with crisis intervention efforts, demonstrating skill in de-escalating tense situations and providing immediate solutions to pressing mental health needs.

• Carry a manageable caseload where you will engage in individual, group, and family therapy sessions, focusing on empowering clients through therapeutic interventions.
\n
• Monitor client admissions, ongoing treatment, and discharge processes, ensuring compliance with standards while addressing barriers to patient care.

• Complete necessary documentation promptly, including reports and data submissions, to maintain accurate records and enhance service delivery.

• Participate in clinical supervision and on-call coverage as assigned, contributing to the overall team success and service quality improvement initiatives.

This position requires sensitivity and understanding to work with a diverse patient population within a hospice setting. Clients may face complex challenges including terminal illnesses accompanied by mental health issues. Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• Current license as a Licensed Clinical Social Worker (LCSW) in Tennessee.

Compensation and Benefits\n

The compensation for this position ranges from USD 70,000 to 85,000 per year, depending on experience and qualifications.
