Key Responsibilities\n

• Develop strategic alliances with community service providers to enhance aftercare arrangements for patients and their families, ensuring they have access to essential resources and support systems.

• Proactively identify and address resistance and conflict during therapeutic processes, implementing effective interventions to reduce tension and guide clients in recognizing unhelpful behaviors and thought patterns.
\n
• Maintain accurate and complete patient records in compliance with organizational standards, documenting assessments, treatment plans, interventions, and communications with families and external agencies.
\n
• Utilize the latest DSM diagnostic criteria to conduct thorough evaluations of children and adolescents, applying knowledge of human development to tailor interventions that meet age-specific needs.

• Implement behavior modification strategies, assertiveness training, and supportive therapy to assist clients in achieving their therapeutic goals.
\n
• Perform comprehensive psychosocial assessments of patients and their families, gathering vital information through effective interviewing techniques and collaboration with the healthcare team.
\n
• Develop and regularly update personalized treatment plans that consider the patient's concerns, strengths, cultural values, and spiritual needs.
\n
• Facilitate educational and therapeutic group sessions, actively documenting patient engagement and feedback to refine group interventions.
\n
• Participate in treatment-planning meetings to discuss patient progress, obtain feedback, and adjust treatment plans as necessary based on ongoing assessments.

Patient Population / Client Focus\n

This position primarily involves working with hospice patients and their families, who are experiencing significant emotional and psychological challenges associated with terminal illnesses. You will provide essential support for navigating grief, loss, and end-of-life decisions, ensuring dignity and respect throughout the care process.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline is required.

• Current and independent licensure as a Licensed Clinical Social Worker (LCSW) in California is mandatory.

• Demonstrated ability to establish and maintain professional boundaries with clients to facilitate trust and therapeutic rapport.

• Strong organizational skills and attention to detail in managing documentation and treatment planning.

• Excellent communication and interpersonal skills, especially in a virtual setting.

• Basic Life Support (BLS) and CPR certification is required for this role.
