Key Responsibilities\n

• Conduct comprehensive intake and psychosocial assessments using standardized tools and personalized inquiry techniques to articulate patients' needs and background.

• Develop, present, and monitor individualized treatment plans based on assessment findings, ensuring alignment with each patient's goals and preferences, and facilitate treatment team discussions for feedback and approval.

• Organize and participate in clinical case conferences, contributing insights and evaluating progress toward treatment goals in a collaborative environment.
\n
• Deliver impactful educational workshops focusing on chemical dependency, employing engaging methods to enhance patient comprehension and participation.

• Provide compassionate and effective counseling services, including one-on-one sessions and group therapy, addressing substance use disorders while fostering a supportive community among attendees.

• Coordinate and manage case management activities, connecting clients to necessary community resources and referrals, ensuring holistic support and follow-through.

Patient Population / Client Focus\n

This role primarily caters to patients within a hospice care setting, focusing on individuals and families facing terminal illnesses. Your expertise will be vital in addressing complex psychosocial challenges, facilitating emotional support, and helping clients navigate end-of-life decisions with dignity and compassion.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a closely related field is required.

• Active and current licensure as a Clinical Social Worker in the state of Pennsylvania.

• A minimum of two years of experience in a mental health or clinical setting, ideally with a focus on hospice or palliative care.

• Strong proficiency in evidence-based therapeutic modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), or Eye Movement Desensitization and Reprocessing (EMDR).

• Excellent assessment and diagnostic abilities, with a track record of developing detailed and actionable treatment plans that reflect the unique needs of each patient.

Compensation and Benefits\n
