Key Responsibilities\n

• Engage actively with participants to preemptively address mental health crises, thereby minimizing reliance on emergency services through proactive care.

• Perform comprehensive assessments and establish personalized care plans that align with each participant's goals and needs.

• Maintain thorough documentation, including crisis alerts and care transitions, to ensure continuous support and coordination with county crisis services and hospital staff.

• Facilitate and document transitions of care for clients moving between hospital and outpatient services, coordinating crucial discharge planning efforts.

• Adapt therapeutic interventions to meet the evolving needs of participants, utilizing evidence-based practices suited to a telehealth format.

• Participate in regular supervision and team meetings to enhance service delivery and share best practices.

Patient Population / Client Focus\n

You will primarily work with individuals facing diverse mental health challenges, including anxiety, depression, trauma, and severe mental illness. Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling.

• An active and valid license as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT).

• Demonstrated ability to manage caseloads effectively while maintaining high standards of care.

• Strong clinical skills, with a proven track record of establishing rapport and trust with clients from diverse backgrounds.
