Key Responsibilities\n

• Conduct thorough, ongoing assessments of patients’ needs, ensuring that care is tailored to meet their individual circumstances.

• Maintain open lines of communication with supervisors, healthcare team members, and clients' family members to provide comprehensive updates on client progress.
\n
• Create and share tentative schedules for appointments and home visits, ensuring efficient monitoring of client work advancement.
\n
• Coordinate a variety of home visits for in-home observations, chart reviews, and interviews to collect and analyze data that inform treatment plans.

• Ensure effective implementation of program plans and actively participate in Interdisciplinary Team Meetings to discuss plans of care.
\n
• Document monthly and quarterly progress notes accurately to maintain a comprehensive record of patient progress and outcomes.
\n
• Lead training sessions to equip fellow team members with necessary skills for implementing program plans and documenting services.
\n
• Continuously provide follow-up support through interviews, home visits, and observations, ensuring clients receive the ongoing care they need.

• Maintain organized records to ensure information accessibility for patients and relevant stakeholders.

• Serve as a liaison with the broader mental health community to advocate for and advance patient care and support community development goals.

• Assist in developing procedural training materials to enhance service delivery and program efficacy.

As a Licensed Professional Counselor with our organization, you will work with a broad spectrum of clients, including children, teenagers, and adults facing various mental health challenges. Qualifications\n

Preferred Experience\n

While 2 years of counseling experience is required, those with a more extensive background in specialized therapy areas or leadership roles will be viewed favorably. Experience in telehealth service delivery is an asset, as it aligns with our primary mode of patient interaction.

Compensation and Benefits\n

This position offers a competitive annual salary ranging from USD 80,000 to 105,000, determined by experience and caseload availability.
