Key Responsibilities\n

• Conduct thorough and comprehensive assessments of clients and their families to identify individual social, emotional, and psychological needs in relation to hospice care.

• Provide individual and family counseling, employing evidence-based therapeutic techniques to support clients as they cope with grief and loss.

• Facilitate group counseling sessions for families, helping them connect with one another and share experiences in a supportive environment.

• Develop tailored service plans based on comprehensive assessments, clearly outlining goals and strategies to meet clients’ unique needs.

• Advocate for clients' access to necessary resources, ensuring they receive adequate support and services as they face the challenges of end-of-life care.

• Contribute to ongoing program development and evaluation, including gathering data for grant reporting and helping to assess the effectiveness of services provided.

• Collaborate with interdisciplinary teams, including medical professionals, to coordinate comprehensive care that addresses clients’ holistic needs.

• Participate in regular team meetings and training sessions, staying current with best practices in social work and hospice care.

Compensation and Benefits\n
