Key Responsibilities\n

• Conduct thorough psychosocial assessments to uncover the emotional, social, and financial needs of patients in hospice care.

• Provide individual counseling and group therapy focused on topics such as grief, adjustment issues, and coping strategies.

• Collaborate with healthcare teams to develop and implement discharge plans, ensuring patients are connected with vital community resources post-discharge.

• Engage in crisis intervention, providing immediate support and resources to patients and their families in distress.

• Educate patients and families about treatment options and available resources, empowering them to make informed decisions about their care.

• Document all interactions and care plans accurately and timely, adhering to legal and hospital policies.

• Participate in hospital committees, offering your expertise on social work matters and advocating for the psychosocial needs of patients.

Patient Population / Client Focus\n

The primary patient population you will serve consists of individuals and families experiencing life-limiting illnesses within the hospice setting. Your role will focus on providing emotional and psychological support during periods of transition, ensuring that patients receive compassionate, comprehensive care that honors their dignity and wishes.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Nevada.

Compensation and Benefits\n
