Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate emotional, social, and environmental factors impacting patients and their families during hospitalization and throughout the hospice experience.

• Formulate and implement tailored intervention plans utilizing evidence-based social work practices to address the unique needs of each patient.

• Facilitate discharge planning and coordination by actively collaborating with the healthcare team, including consulting physicians and other specialists, to ensure smooth transitions for patients.

• Engage with patients and their families, providing education and counseling that addresses adaptation to illness and end-of-life considerations.

• Utilize individual, group, and family therapies as needed, including crisis intervention, to support patients in managing anxiety, depression, and other emotional challenges related to their health conditions.

• Provide training and guidance to other healthcare professionals on psychosocial oncology practices, enhancing the overall competency of the team.

• Contribute to community outreach programs that provide education and promote resources and support for cancer survivors and their families.

• Participate in on-call rotations to respond to urgent patient needs after hours, ensuring continuity of care.

• Present at educational conferences as a recognized expert in psychosocial oncology practice, sharing insights and best practices with fellow professionals.

Patient Population / Client Focus\n

The primary focus of this role is on patients experiencing terminal illness and their families, particularly those receiving hospice care. You will work with a diverse patient population that includes individuals with varying diagnoses, ensuring that their emotional and psychosocial needs are addressed through individualized care approaches.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution required.

• Current and active Licensed Clinical Social Worker (LCSW) license in the state of New Jersey.

• Experience in a healthcare, hospice, or palliative care setting preferred.

Compensation and Benefits\n

The position offers a competitive salary range of USD 70,000 – 100,000 annually, commensurate with experience and qualifications.
