Key Responsibilities\n

• Participate in Interdisciplinary Team Meetings to strategize collectively on client care and share insights.

• Document insights and developments through monthly and quarterly progress notes, contributing to comprehensive care plans.

• Organize training sessions to enhance team competency in program implementation and documentation practices.

• Provide ongoing follow-up care through interviews and home visits, ensuring sustained support for clients' needs.

• Maintain accurate records to facilitate transparency and easy access to relevant information for stakeholders.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, Social Work, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in New Jersey.

Compensation and Benefits\n
