Key Responsibilities\n

• Conduct thorough psychosocial assessments by engaging in meaningful conversations with patients and their families, assessing emotional, social, and environmental factors that affect recovery.

• Offer counseling sessions aimed at addressing adjustment issues, grief, and emotional wellbeing, equipping families with coping strategies and resources.

• Collaborate closely with the rehabilitation team, which includes physical and occupational therapists and healthcare professionals, to develop individualized treatment plans that reflect patients’ unique psychosocial needs.

• Guide patients in accessing essential community resources, such as skill-building workshops, support groups, and employment assistance, ensuring they have the tools they need for post-rehabilitation success.

• Play an integral role in discharge planning, facilitating seamless transitions for patients transitioning to home care or other care settings.

• Assist in creating educational materials for families regarding available post-rehabilitation services that enhance long-term success in recovery.

Patient Population / Client Focus\n

In this role, you will work with a varied patient demographic facing life-limiting illnesses, including those dealing with strokes, spinal cord injuries, and other significant health conditions. Your expertise as a Licensed Clinical Social Worker will not only focus on mitigating their challenges but also help in enhancing their overall quality of life during critical times. Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from $75,000 to $100,000 annually, commensurate with experience and qualifications. In addition to a competitive salary, we offer a substantial sign-on bonus as an incentive to join our passionate team.
