Key Responsibilities\n

• Conduct in-depth counseling sessions addressing both substance use disorders and co-occurring mental health challenges for a diverse clientele, including individuals and couples.

• Develop personalized treatment plans and case management strategies aimed at achieving measurable outcomes, guiding clients through their recovery journey.

• Maintain meticulous documentation of client interactions and treatment progress within our client management system, ensuring compliance with regulatory standards.

• Coordinate ongoing appointments, managing a dynamic calendar to accommodate clients’ needs and facilitate continuous support.

• Engage in regular team meetings to discuss case strategies, share insights, and develop collaborative approaches to client care.

Patient Population / Client Focus\n

You will work with a diverse client base, focusing on youth, adults, and families referred to our hospice services. Qualifications\n

• A Bachelor’s Degree in Human Services or a related behavioral healthcare field.

Compensation and Benefits\n
