Work Environment

This position is primarily onsite in patients' homes throughout Oakland, California. As a Home Care Physical Therapist, you will have the unique opportunity to work closely with patients in their environments, tailoring treatment plans to their specific needs and goals. Key Responsibilities

• Evaluate and assess patients referred by physicians to develop a comprehensive treatment plan that aligns with their individual health objectives.
• Implement evidence-based therapeutic interventions to improve patients' functional abilities across a spectrum of conditions.
• Maintain high standards of customer service, ensuring a positive experience for each patient while demonstrating empathy and understanding.
• Collaborate with multidisciplinary teams to optimize treatment approaches, leveraging the expertise of colleagues and ancillary services to enhance patient care.
• Conduct therapy sessions in patients' homes, utilizing state-of-the-art techniques and equipment to promote effective recovery.

Patient Population / Client Focus

As a Physical Therapist at Optimal Recovery, you will work with a diverse array of patients, including those with orthopedic, pediatric, neurologic, and sports-related injuries. Qualifications

• A Bachelor's Degree in Physical Therapy from an APTA or CAPTE-accredited program.
• A valid and current state license to practice physical therapy in California.
• Basic Life Support (BLS) certification from the American Heart Association.
• Excellent interpersonal and communication skills, both written and verbal, to establish rapport with patients and their families.
• Proficiency in word processing and electronic documentation systems.
• Ability to foster strong, productive relationships with patients and community members, encouraging their involvement in the recovery process.
• A commitment to professional growth, with a keen desire to advance clinical expertise and maintain up-to-date knowledge of best practices in physical therapy.

Preferred Experience

• Experience in home health settings, particularly with patients requiring interdisciplinary support.
