Work Environment

As a Full-Time Physical Therapist in our home care setting, you will work onsite and provide treatment directly in patients' homes, allowing for personalized care in a comfortable environment. Key Responsibilities

Your primary responsibilities will include:

• Conducting thorough patient evaluations and assessments to identify functional limitations and develop effective treatment plans tailored to individual needs.
• Implementing therapeutic interventions and continuously monitoring progress, adjusting treatment as necessary to ensure optimal outcomes.
• Educating patients and their families about therapy techniques, exercises, and home health practices that support recovery and ongoing wellness.
• Maintaining accurate, timely documentation of patient treatments and outcomes, in compliance with all relevant laws and professional guidelines.
• Engaging in collaborative discussions with other healthcare professionals regarding the overall care and management of each patient's rehabilitation journey.
• Participating in community outreach initiatives to promote health and wellness and to provide awareness of the services offered by Amedisys.
• Supervising and mentoring support staff to enhance team performance and patient care delivery.

Qualifications

To be considered for this position, candidates must possess:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A current and valid State License in Physical Therapy.
• Excellent written and verbal communication skills, essential for educating patients and collaborating with healthcare teams.
• Competence in word processing and various computer applications, crucial for documentation and communication purposes.

Preferred Experience
