Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to understand the emotional, social, and financial needs of patients, enabling tailored interventions.

• Provide individual and group counseling sessions that address various issues, including adjustment disorders, grief, and coping strategies for managing illness.

• Collaborate closely with interdisciplinary medical teams to ensure cohesive and integrative care plans that consider the emotional and psychological needs of patients.

• Develop and execute discharge plans, connecting patients with community resources and post-discharge support systems to promote successful transitions.

• Offer educational sessions and emotional support to patients and their families regarding available treatment options and recovery resources.

• Participate actively in hospital committees focused on quality improvement initiatives, contributing your expertise on social work matters.

• Document all assessments, interventions, and progress notes in compliance with organizational policies, legal standards, and ethical guidelines.

• Provide crisis intervention and support, ensuring timely assistance when patients face acute psychological distress.

In this role, you will primarily work with a diverse patient demographic that includes individuals from various socioeconomic backgrounds, age groups, and cultural identities.
