Work Environment

This position is primarily onsite, providing care directly in the homes of our patients. Key Responsibilities

• You will conduct comprehensive evaluations, gathering pertinent history and performing specific assessments to determine individual patient needs.
• Effective communication is vital; you will explain evaluation findings and develop treatment plans in partnership with patients and their families, incorporating their goals and preferences.
• Administer treatment interventions based on detailed evaluations, tailoring approaches to meet the specific presentations of patients and adapting protocols as needed in response to their progress.
• Collaboration with multidisciplinary healthcare teams is essential; you will consult with physicians and other providers as indicated, documenting any changes in patients' conditions that may necessitate adjustments to their care plans.
• Complete discharge planning and follow-up to ensure continuity of care, accurately documenting all encounters and maintaining a detailed record of each patient's journey through therapy.
• Exhibit strong time management skills to meet the productivity expectations while maintaining an accurate schedule and participating actively in community marketing initiatives.
• Take an active role in departmental goal setting and compliance with relevant healthcare regulations, contributing to a collaborative and effective therapeutic environment.
• Engage in personal and professional development opportunities to enhance skills and maintain a high level of competency.
• Promote a positive patient experience, demonstrating flexibility and adaptability to meet diverse needs.

Patient Population / Client Focus

In this role, you will primarily work with a diverse patient population, including individuals recovering from surgery, those with chronic conditions, and elderly clients requiring rehabilitation services. Tailoring your approach according to the unique circumstances of each patient, you will focus on enhancing mobility, alleviating pain, and promoting overall physical function, thus significantly contributing to their quality of life.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
• A valid state license to practice physical therapy, as required by Virginia regulations.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• While a minimum of two years of experience is preferred, we encourage new graduates with a strong foundation in physical therapy principles to apply.

Preferred Experience

We highly value clinical experience in outpatient settings or home care, as well as familiarity with electronic health record systems and patient management software. Experience in collaborative healthcare environments and commitment to patient-centered practices will be advantageous.
