Key Responsibilities\n

• Conduct thorough assessments to identify the social, emotional, and environmental challenges faced by clients, utilizing standardized tools and interviews.

• Develop, implement, and monitor individualized service plans that include measurable goals, interventions, and regular progress evaluations.

• Provide one-on-one counseling and family therapy to assist clients in navigating life transitions and personal crises.

• Collaborate with clients to create personalized service plans, linking them to vital community resources, including housing, health care, and employment services.

• Offer immediate crisis intervention, risk assessments, and safety planning for clients in distress.
\n
• Work closely with community organizations and agencies to facilitate cohesive service delivery and advocate for clients' needs.

• Engage in community outreach initiatives aimed at raising awareness of our services and addressing local health disparities.

• Regularly participate in case conferences, training sessions, and team meetings to enhance collaboration and professional development.

Patient Population / Client Focus\n

Your primary focus will be on individuals and families experiencing substance abuse challenges, often compounded by cooccurring mental health issues. Working as part of the Detox Center team, you will provide essential support services to those in crisis, promoting their journey towards recovery and stability.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required, demonstrating a foundation of knowledge in therapeutic practices.

• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory for this role.

• Excellent communication, cultural competence, and problem-solving skills are essential, as you will engage with diverse populations.

• Knowledge of community resources, social and economic justice issues, and trauma-informed care will greatly enhance your effectiveness in this role.
