Wide Stream Pain Solutions is seeking a dedicated and knowledgeable Nurse Practitioner to join our outpatient healthcare team in Ossining, New York. Work Environment

This position is based onsite, primarily in a home care setting, allowing for patient-centered interactions that ensure personalized care. The typical work schedule is Monday through Friday, totaling 40 hours per week, with some rotating Saturday coverage, allowing for a balance between professional responsibilities and personal time.

Key Responsibilities

As the Nurse Practitioner, you will play a vital role in managing both the physical and psychological aspects of patient care. Your specific responsibilities will include:

• Conducting comprehensive assessments to identify normal and abnormal findings related to cardiovascular health.
• Developing and implementing individualized treatment plans based on patient needs, followed by continuous reassessment and adjustments as necessary.
• Evaluating patient responses to medications and treatments, ensuring optimal therapeutic outcomes.
• Prescribing immunizations according to established schedules, contributing to preventive health measures.
• Fostering effective communication with fellow healthcare team members to ensure coordinated patient care and seamless transitions between services.
• Maintaining meticulous documentation of problem lists and medication records at each patient encounter to ensure accurate care continuity.
• Adhering to compliance regulations to ensure documentation is complete, accurate, and timely.

Patient Population / Client Focus

You will primarily interact with patients experiencing cardiovascular conditions, ranging from chronic heart disease to those needing interventional procedures. Through empathetic care, you will help patients understand their conditions and treatment regimens, fostering a supportive atmosphere that promotes adherence to medical recommendations.

Qualifications

• A master's degree in Nurse Practice from an accredited program.
• Current Registered Nurse (RN) license issued by the state of New York.
• Basic Life Support (BLS) certification is mandatory.

Preferred Experience

While two years of general nurse practitioner experience is required, we highly value candidates with 2-3 years of experience specifically in cardiology. Compensation and Benefits
