Key Responsibilities\n

• Conduct ongoing assessments to identify and address the unique needs of each patient effectively.

• Facilitate communication among supervisors, family members, guardians, and relevant parties to monitor and report on individual progress.

• Create and deliver a tentative schedule for appointments, home visits, and monitoring observations, ensuring efficient time management.
\n
• Coordinate home visits and observations to adhere to program requirements, conducting thorough chart reviews and staff interviews as necessary.
\n
• Oversee the implementation of the program plan, ensuring it is executed effectively and aligns with patient needs.
\n
• Actively participate in Interdisciplinary Team Meetings to share insights and collaborate on patient-centered care plans.
\n
• Prepare monthly and quarterly progress notes, documenting patient advancements and areas needing attention.
\n
• Organize training sessions to ensure staff competency in program implementation and compliance with documentation standards.
\n
• Provide follow-up services, including interviews and home visits, to continue supporting patient development and addressing ongoing needs.

• Maintain accurate records and documentation to ensure relevant information is accessible to all stakeholders.

• Serve as a liaison between the mental health community and the public, promoting mental wellness initiatives and program goals.

• Contribute to the development of training materials that enhance our programs for individuals seeking counseling services.

Qualifications\n

• Possession of a Master’s or Doctoral degree in Counseling, Psychology, or a related mental health field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon.
