Key Responsibilities\n

• Develop and present a detailed schedule of appointments, home visits, and observational tasks to supervisors to monitor treatment efficacy.

• Coordinate home visits, which involve comprehensive data analysis, reviews of client files, and interviews with staff to ensure robust support for clients.

• Implement the individualized program plans effectively, ensuring that each client receives the best possible care.

• Actively participate in Interdisciplinary Team Meetings and contribute to case discussions to enhance collaborative patient care.

• Document monthly and quarterly progress notes, maintaining clear records to facilitate transparent communication and treatment improvements.

• Organize and facilitate training sessions to ensure staff competencies align with best practices in program implementation and documentation.

• Provide ongoing support through follow-up interviews and home visits, assisting clients in reaching their mental health goals.
