Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions and needs in a home care setting, implementing personalized treatment plans based on individual goals.
• Utilize diverse therapeutic techniques, such as manual therapy, exercise regimens, and modern rehabilitation technologies, to alleviate pain and restore mobility.
• Develop and execute individualized treatment plans, closely tracking and documenting patients' progress and responses to therapy.
• Regularly reassess patients and modify treatment strategies accordingly to meet evolving therapeutic objectives, ensuring optimal outcomes.
• Recommend and fit patients with appropriate assistive devices for mobility and self-care, demonstrating proper usage to patients and their families.
• Provide educational support and training for patients, caregivers, and other healthcare team members on physical therapy practices and assistive technologies.
• Maintain professional knowledge and expertise through ongoing education, participation in workshops, and engagement with professional organizations.
