Key Responsibilities\n

• Conduct in-depth assessments and provide counseling to individuals, families, children, youth, and adults, focusing on their psychosocial needs and mental health challenges.

• Collaborate with the practitioner team to identify and de-escalate potential risks among clients, providing immediate support and intervention as necessary.

• Lead efforts to escalate critical issues to emergency services and ensure adherence to mandatory reporting requirements when necessary.

• Identify and communicate new safeguarding risks to Clinical Leads, contributing to effective risk mitigation strategies.
\n
• Stay informed about the latest safeguarding policies and procedures, integrating them into daily service delivery while proactively addressing implementation challenges.

• Monitor outcome-focused practices, gather feedback from clients and families, and recommend adjustments to services to enhance overall effectiveness and satisfaction.

Patient Population / Client Focus\n

In this role, you will primarily serve clients facing psychosocial challenges associated with serious and terminal health conditions. Your work will involve addressing the unique emotional, social, and mental health needs of hospice patients and their families, ensuring they receive holistic support throughout their care journey. You will also assist those with substance use challenges, providing critical interventions and resources to help them navigate their recovery processes.

Qualifications\n

• Education: A Master's Degree in Social Work from an accredited program is required for this role.

• Licensure: Must possess a valid and active Licensed Clinical Social Worker (LCSW) certification.

• Experience: A minimum of two years of experience working in a behavioral health setting, with a strong emphasis on both case management and direct patient care.

• Skills: Proficient in taking comprehensive psychosocial histories, conducting case management evaluations, performing mental health assessments, including suicide risk assessments, and utilizing effective interview techniques.

• Experience with hospice or palliative care environments is highly desirable, as well as familiarity with the unique challenges faced by clients in these settings.

Compensation and Benefits\n

The position offers a competitive salary range of USD 90,000 to 100,000 annually, commensurate with experience and qualifications.
