You'll be part of the therapy specific team supporting Home and Heart Home Health Care Agency, working within a collaborative, multidisciplinary model. Care is coordinated, patient-centered, and team-driven, with shared accountability and open communication.

You're supported not just as a clinician, but as a professional with long-term goals. Opportunities include participation in the TPS Clinical Excellence Program, offering access to mentorship, advanced training and certifications. Job Overview & Work Site

You'll work with a diverse caseload, including a higher-than-average post-operative orthopedic population.

You're set up to do your best work through:

• Thoughtful scheduling and defined territories to support appropriate caseloads
• Limited drive time between visits whenever possible
• Clinical leadership from therapists who understand day-to-day practice demands
• Individualized support aligned with your clinical interests and goals
• Collaboration with Home and Heart healthcare colleagues for a team-based approach to care

License & Experience

Requirements for this position include the following:

• Graduate (current or pending) from an accredited physical therapy program
• Active or pending state physical therapist license
• Recent graduates encouraged to apply
