Registered Nurse (RN) – Home Health Location: Havertown, Pennsylvania. Job ID: JR255633. Responsibilities As a home health registered nurse case manager, you will lead and collaborate to manage and deliver patient care plans, ensuring high-quality care for patients in Havertown and surrounding areas. Key duties include: Perform patient care visits and non‑visit activities. Document patient care and chart critical deadlines consistently. Use tablets, mobile phones, and EMR software for documentation and communication. Manage your time and schedule effectively. Support the 24/7 after‑hours care team when needed. Qualifications RN license in the state where you will work. Graduate from an approved practical nursing school. Minimum one year of RN experience with clinical competence. Current driver’s license and ability to spend ~20% of your day driving to patient locations. Demonstrated experience with tablets, mobile phones, and EMR software.
