Job Overview Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to support patients in Downers Grove, IL and surrounding areas. This full‑time position is ongoing, 40hours per week, 8‑hour shifts with a 4‑month orientation pay guarantee. Key Responsibilities Lead and collaborate to provide management and delivery of patient care plans as a home health registered nurse case manager. Provide quality patient care and maintain timely charting to meet critical deadlines. Self‑manage time and schedule, ensuring consistent availability for patient visits. Use tablets, mobile phones, and EMR software to document and coordinate care. Qualifications Current RN license in the state of employment. Graduate from an approved school of practical nursing.
