Work Environment\n

This position offers a remote working arrangement predominantly focused on the school setting, allowing for a flexible and dynamic work environment. The LCSW will engage with students, faculty, and families and will participate in multidisciplinary team meetings. Although remote, travel will be required to various CHDC locations for meetings, training, and outreach activities, promoting interconnectivity and collaborative care approaches among health professionals.

Key Responsibilities\n

The LCSW will engage in a variety of responsibilities that uphold the mission of CHDC while ensuring high standards of care are met. Specific duties will include:

• Providing personalized support to mental health consumers as they navigate both physical and behavioral healthcare systems to ensure that integrated health care solutions are effectively delivered.

• Educating students and their families on available resources and services, with the aim to enhance their engagement in health-promoting behaviors, disease prevention initiatives, and overall well-being.
\n
• Active participation in multidisciplinary treatment team meetings to develop comprehensive treatment plans.
\n
• Timely reporting of crises or unusual incidents to the Medical Director and immediate reporting of psychiatric and medical emergencies to the Chief Medical Officer (CMO).
\n
• Maintaining thorough documentation in clinical records and ensuring compliance with organizational procedures and policies.
\n
• Participating in Quality Improvement initiatives and attending designated meetings and training sessions to enhance service delivery.
\n
• Administering evidence-based screening tools to assess the healthcare needs of students and documenting outcomes.
\n
• Collaborating closely with the Center of Excellence Team in managing cases involving Opioid Use Disorder.

Patient Population / Client Focus\n

This role primarily serves students within local schools, especially those facing mental health challenges and socio-economic barriers. The LCSW will work diligently to establish trusting relationships with clients and stakeholders, focusing on delivering culturally competent care that acknowledges the unique backgrounds and experiences of each individual.

Qualifications\n

To qualify for this vital role, candidates must meet the following criteria:

• A master’s degree in social work is required, along with an active Licensed Clinical Social Worker (LCSW) credential.

• At least 2 to 3 years of relevant experience in social work, ideally in a medical setting, such as primary or specialty care.

• A proven ability to work effectively in a collaborative team environment, providing support to clients and working alongside other healthcare professionals and social service providers.

• Willingness to travel to CHDC sites, including participation in off-site meetings, retreats, and outreach events as needed.

Preferred Experience\n

While not mandatory, candidates who possess experience working in school settings and with diverse populations will be favored. Familiarity with evidence-based practices and specific knowledge of mental health issues impacting adolescents will also enhance the candidate's applicability for this position.

Compensation and Benefits\n

CHDC offers a competitive salary range of USD 90,000 to 100,000 annually, commensurate with experience.
