Key Responsibilities\n

As an LCSW at CHDC, your responsibilities will include:

• Assisting mental health clients in navigating both physical and behavioral healthcare systems to foster integrated health management.

• Educating patients on available resources and services, advocating for engagement in wellness programs, disease prevention, and overall health improvements.

• Actively participating in multidisciplinary treatment team meetings to collaboratively manage patient care strategies.
\n
• Reporting all crises and unusual incidents promptly to the Medical Director, with immediate notification of psychiatric and medical emergencies to the Chief Medical Officer (CMO).

• Accurately documenting clinical records and maintaining detailed notes on treatment plans and patient progress.
\n
• Engaging in quality improvement initiatives as indicated, attending all required meetings and ongoing training sessions.
\n
• Adhering strictly to all organizational policies and procedures, contributing to a culture of accountability and ethical practice within integrated primary care behavioral health models.

• Implementing evidence-based screening tools to evaluate the healthcare needs of patients and providing timely feedback on findings.
\n
• Collaborating with our Center of Excellence Team to offer specialized support and treatment for patients with Opioid Use Disorder.

Patient Population / Client Focus\n

In this role, you will work with a diverse patient population, including individuals from various socioeconomic backgrounds, ethnicities, and lifestyles. Many of our patients may face significant barriers to accessing healthcare, which can include mental health challenges, substance use disorders, and chronic medical conditions. Qualifications\n

• A Master’s degree in Social Work from an accredited program, with current LCSW licensure in Pennsylvania being mandatory.
