Key Responsibilities\n

• Conduct comprehensive assessments to identify the mental health needs of clients within your designated Care Pod.
\n
• Develop and implement tailored treatment plans that reflect individual patient needs and therapeutic goals.
\n
• Engage in regular collaboration with the patient’s primary care provider and other relevant service providers, ensuring a holistic approach to behavioral health care.
\n
• Monitor and document changes in clinical symptoms and treatment side effects, working towards mitigating issues and improving patient outcomes.
\n
• Facilitate medication management support in collaboration with prescribers, focusing on adherence, complications, and overall treatment effectiveness.
\n
• Utilize our patient registry system to track clinical outcomes, maintain accurate records of encounters, and identify patients needing re-engagement during follow-up.
\n
• Set specific, actionable self-management goals in partnership with patients, empowering them in their recovery journey.
\n
• Proactively adjust care plans in response to patient progress or lack thereof, including recommending medications or psychosocial interventions and making referrals for additional support services.

• Collaborate with care teams to establish agreed-upon management plans with the patient and their families, ensuring clarity and mutual understanding of care approaches.

Patient Population / Client Focus\n

You will work with a diverse clientele encompassing individuals, couples, and families seeking assistance with a myriad of emotional and psychological challenges. This may include anxiety, depression, relationship issues, trauma, and other behavioral health concerns. Understanding and approaching each patient from a culturally competent standpoint will be key to building rapport and trust, ultimately aiding in their therapeutic success.

Qualifications\n

• An active LCMFT license specifically in the state of Maryland is a must.

• A Master’s or Doctoral degree in Marriage and Family Therapy or a closely related field is required.

Compensation and Benefits\n

We offer a competitive salary range of $80,000 to $90,000, commensurate with experience and qualifications. Employees may also qualify for health benefits, flexible scheduling options, and additional perks including tuition reimbursement for ongoing education.
