Work Environment\n

As a licensed master social worker at our facility, you will have the flexibility of a remote work environment while engaging with clients primarily through telehealth services. This allows for significant interaction while also ensuring the safety of our clients, especially in the context of public health considerations. You will be part of a collaborative interdisciplinary team that includes healthcare providers, behavioral health specialists, and caregivers who work together to create comprehensive care plans for clients.

Key Responsibilities\n

Your day-to-day responsibilities will include, but are not limited to:

• Conducting comprehensive assessments of seniors’ psychosocial well-being, taking into account emotional, mental, and social elements that affect their daily lives.

• Developing tailored care plans in collaboration with clients and their families, aligning with their individual goals, preferences, and cultural backgrounds.

• Providing counseling and emotional support during challenging life transitions, facilitating effective coping strategies and solutions.

• Advocating for clients’ rights, ensuring they have access to vital community resources, healthcare services, and support programs.

• Maintaining close coordination with medical professionals and caregivers to monitor client progress, updating care plans as needed to reflect changes in their condition.

• Educating families about available resources and support groups that could enhance their loved one’s social engagement and overall health.

• Documenting all interactions and client progress comprehensively and accurately within electronic health record (EHR) systems.

Patient Population / Client Focus\n

This role primarily focuses on seniors who may be experiencing various emotional and social challenges including mental health issues, social isolation, cognitive decline, or transitional challenges related to aging. You will work to promote their dignity and autonomy while engaging their families in the care process to ensure holistic and effective support.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid Texas state licensure as a Licensed Master Social Worker (LMSW).
\n
• Minimum of two years of experience in social work, particularly within community organizations or geriatric care settings.

• Strong understanding of social work ethics, principles, and practices, particularly relating to aging populations.

• Ability to communicate effectively and empathically, establishing rapport with both seniors and their family members.

• Proficient in using electronic health records and technology to maintain documentation and engage in remote client communication.

• Strong crisis intervention and conflict resolution skills.

Compensation and Benefits\n
