Key Responsibilities\n

The Licensed Clinical Professional Counselor will be responsible for:

• Conducting comprehensive patient intakes focusing on diagnostic evaluations and making informed recommendations to primary care providers regarding treatment plans.

• Providing brief and focused therapeutic interventions for patients in need of mental health support, including techniques aligned with the Federally Qualified Health Center (FQHC) guidelines.

• Advising primary care teams on patient needs and determining the appropriate level of care, including referrals to specialized mental health facilities when necessary.

• Developing and maintaining relapse prevention plans for patients requiring ongoing support to ensure stable functioning and preventive care.
\n
• Monitoring patient progress and effectiveness of treatment, while making adjustments as needed based on individualized assessments.
\n
• Documenting detailed patient progress notes, treatment plans, and relevant diagnostic information in accordance with clinical standards and organizational policies.
\n
• Engaging in interdepartmental collaboration to align behavioral health initiatives with primary care services.
\n
• Offering guidance on health behaviors to assist patients in making lifestyle changes conducive to their overall mental and physical health.

• Participating actively in clinical meetings and contributing to service evaluation aimed at improving behavioral health outcomes.

Qualifications\n

To be successful in this role, applicants should have:

• Current licensure as a Licensed Clinical Professional Counselor (LCPC) and a minimum master's degree in Clinical Counseling or a closely related field.

• An understanding of developmental psychopathology and experience in family therapy, group therapy, or child psychotherapy.

Preferred Experience\n

While not required, candidates with bilingual proficiency in English and Spanish will find that their skills are particularly valuable for serving our diverse patient population. Previous telehealth experience and familiarity with electronic health record (EHR) systems will also be advantageous in this role.

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 87,000 to 105,000 annually, depending on experience and qualifications.
