Key Responsibilities\n

• Act as a liaison between clients, families, and outpatient providers to facilitate comprehensive care plans.

• Conduct thorough assessments, offering diagnostic evaluations to determine clients' needs and preferences.

• Interface with insurance providers for pre-certification and utilization management, streamlining authorization processes.

• Participate actively in discharge and aftercare planning, ensuring continuity of care for clients transitioning from intensive treatment.

• Provide therapeutic meal support, striving to model and promote healthy relationships with food in alignment with therapeutic goals.
