Key Responsibilities\n

• Collaborate with a multidisciplinary team of healthcare professionals—including physicians, nurses, and health educators—to assess and address complex client needs, ensuring that all care plans are practical and tailored to individual circumstances.

• Develop and manage individualized treatment plans by conducting thorough biopsychosocial assessments, setting measurable goals, and diligently monitoring progress toward said goals.

• Educate clients and their families about available mental health resources, including community programs and self-help strategies, ensuring a clear understanding of treatment processes and support options.

• Utilize evidence-based practices and stay current with the latest mental health research, offering informed recommendations for treatment interventions and resources.

• Document all client interactions and treatment progress accurately in electronic health records, ensuring compliance with privacy regulations and internal policies.

• Provide mentorship and support to less experienced team members, sharing knowledge and resources to enhance their clinical skills and therapeutic approaches.

Patient Population / Client Focus\n

The clients served at our community health clinic predominantly include elderly populations facing challenges such as chronic illness, social isolation, and mental health disorders. Qualifications\n

• Master’s degree in Social Work from an accredited program.

• Current and unrestricted Licensed Clinical Social Worker (LCSW) license in the state of Nevada.

• Strong clinical skills with at least 2 years of post-licensure experience in a mental health or clinical setting.
