Key Responsibilities\n

• Utilize your education and training to conduct thorough psychosocial assessments and formulate independent diagnoses tailored to each patient’s unique circumstances.

• Collaborate with patients to create personalized, patient-centered treatment plans that address their specific mental and emotional health challenges.

• Deliver evidence-based behavioral interventions, employing techniques such as motivational interviewing, solution-focused approaches, and behavioral activation, ensuring that treatment is both effective and adaptable to patient needs.

• Coordinate and communicate with other healthcare professionals, including primary care providers and psychologists, to support a well-rounded approach to patient care, fostering a collaborative healthcare environment.

• Regularly monitor patients through structured follow-up sessions, assessing their progress towards established treatment goals and making adjustments to their treatment plans as necessary.

• Assist in managing psychotropic medications as prescribed by medical providers, focusing on adherence to treatment, side effects, and overall effectiveness, while keeping detailed records of medication interactions and patient feedback.

As a Licensed Clinical Social Worker at Mindful Meditations Wellness Center, you will work with a diverse client population, including children, adolescents, adults, and older adults facing a wide range of mental health issues. Qualifications\n

Compensation and Benefits\n
