Work Environment

This full-time position operates entirely onsite, directly within clients' homes. Our Home Care setting allows for a unique therapeutic relationship as therapists work one-on-one with patients, offering them comfort and the personalized attention they need to rebuild strength and functionality. Key Responsibilities

• Conduct thorough evaluations of patients upon initial visits to assess their condition and needs related to recovery from injuries, surgeries, or chronic ailments in a home care setting.
• Develop and implement individualized treatment plans based on evaluations, integrating evidence-based practices such as targeted exercises, manual therapy techniques, and mobility training to facilitate patients' recovery.
• Regularly monitor and document patient progress, adjusting treatment plans as necessary to meet evolving goals and improve overall health outcomes.
• Educate patients, caregivers, and families on rehabilitation exercises, the use of assistive devices, and best practices for home care to maximize independence.
• Collaborate with other healthcare professionals to ensure a holistic approach to patient wellness, sharing insights and recommendations for comprehensive care.
• Stay abreast of advancements in physical therapy practice through continuous education, participation in workshops, and engaging in professional organizations.

Qualifications

• Possession of a Doctorate in Physical Therapy (DPT) from an accredited institution.
• Valid State License to practice Physical Therapy in New Jersey.

Compensation and Benefits
