Work Environment

This role is entirely onsite, requiring you to visit patients in their homes. You will collaborate closely with our multidisciplinary team of healthcare professionals, ensuring that patients receive holistic care in a comfortable and familiar environment. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians, assessing their physical capabilities and limitations.
• Develop and implement individualized treatment plans based on patient evaluations and physician directives, focusing on restorative and rehabilitative techniques.
• Utilize evidence-based practices and the latest therapeutic modalities to enhance recovery, which may include manual therapy, therapeutic exercises, and mobility training.
• Regularly monitor and document the progress of patients, making necessary adjustments to treatment plans to ensure effectiveness and adherence to the physician's plan of care.
• Provide education and training to patients and their caregivers on exercises, techniques, and lifestyle modifications to foster ongoing recovery.
• Demonstrate exceptional customer service by engaging with patients to build rapport and trust, promoting a positive therapeutic environment.
• Collaborate with other healthcare professionals to maximize treatment outcomes, participating in team meetings to discuss patient progress and share insights.
• Meet or exceed minimal utilization goals and business objectives while maintaining high-quality care standards.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population including those recovering from orthopedic surgeries, neurological conditions, pediatric patients, and sports-related injuries. Your expertise will guide patients through their recovery journey, ensuring their unique needs are met through tailored treatments in the comforts of their homes.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current state license as a Doctor of Physical Therapy (DPT) issued by the Physical Therapy Examining Committee.
• Minimum of two years of clinical experience in physical therapy.
• Current CPR certification to ensure the safety and well-being of our patients.
• Strong verbal and written communication skills; capability in word processing and relevant software.
• Ability to develop strong therapeutic relationships with patients to understand and exceed their functional goals.
• A genuine desire to connect positively with patients and deliver high standards of care.

Preferred Experience

While a minimum of two years' experience is required, additional experience working in home care settings or with specialized populations such as pediatrics or geriatrics is advantageous. Familiarity with electronic health records (EHR) systems and telehealth services is also beneficial.

Compensation and Benefits

We offer a competitive salary range between USD 100,000 and 120,000 per year, complemented by a comprehensive benefits package.
