Work Environment

This role is performed onsite, visiting patients in their homes throughout Highland Park and surrounding areas. As a home care therapist, you will have the unique opportunity to assess and treat patients in the comfort of their own environments, taking into consideration their daily routines, family dynamics, and living conditions. This setting allows for a more personalized approach to therapy, where you can track progress in real-life scenarios.

Key Responsibilities

• Consult with other healthcare professionals as necessary, recognizing and documenting changes in patients' conditions that may require modifications to the plan of care.
• Conduct thorough evaluations, including the assessment of patient histories, physical capabilities, and functional limitations, while providing clear recommendations and thorough documentation of findings.
• Implement evidence-based treatment protocols tailored to the specific needs of each patient, utilizing advanced techniques and technologies.
• Complete all necessary discharge planning and follow-up procedures, ensuring accurate and timely documentation of patient care activities.
• Maintain compliance with corporate, clinical services training, and all state/local/federal regulations, ensuring high standards of patient safety and care.
• Foster a supportive environment through respectful communication among co-workers, physicians, patients, families, and insurance payors.
• Engage in facility meetings and professional development opportunities to enhance team collaboration and personal growth.

Patient Population / Client Focus

Your primary focus will be on a diverse range of orthopedic patients who are in need of rehabilitation services due to surgeries, injuries, or chronic conditions. You will provide treatment aimed at restoring mobility, reducing pain, and improving overall patient function. This role requires not only clinical expertise but also the ability to empathize and connect with individuals as they navigate the challenges of recovery.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; a Doctorate degree (DPT) is preferred.
• Current license to practice as a Physical Therapist in the state of New Jersey.
• Certification in Basic Life Support (BLS) from the American Heart Association is mandatory.

Preferred Experience

• Minimum of 2 years of experience in a clinical setting, preferably within home care or outpatient rehabilitation.
• Experience with orthopedic patients and familiarity with relevant treatment techniques.

Compensation and Benefits

The position offers a competitive salary range between USD 100,000 and 120,000 per year, commensurate with experience and qualifications. In addition to the base salary, Headstrong Rehabilitation & Wellness provides a comprehensive benefits package that includes:
