Work Environment

This position is primarily onsite, where you will visit patients in their homes across the Old Bridge area. Our home care model is designed to provide tailored therapeutic interventions in a familiar setting, which often leads to increased patient comfort and motivation. Key Responsibilities

• Conduct thorough evaluations of patients' physical health by reviewing medical histories, performing specific assessments, and determining treatment recommendations that meet individual needs.
• Communicate evaluation findings and treatment plans effectively to patients and their families, involving them in collaborative goal-setting to enhance engagement in their recovery journey.
• Implement evidence-based treatment protocols, adjusting your approach as necessary based on ongoing assessments and the evolving healthcare landscape to ensure optimal patient outcomes.
• Consult with other healthcare professionals when necessary and document any changes in the patient's condition to adapt treatment plans promptly.
• Manage discharge planning, ensuring patients have a clear understanding of their follow-up care, while maintaining precise records of evaluations, treatment progress, and discharge summaries.
• Demonstrate exceptional time management skills and consistently meet productivity expectations while maintaining a precise patient schedule.
• Engage in marketing activities that promote Aviant Physical Therapy within the community, enhancing access and awareness of our home care services.
• Set personal and departmental goals that align with Aviant's mission and values, actively participating in initiatives that ensure compliance with regulatory standards.
• Take initiative for your continuous professional growth, mentoring future physical therapists as a clinical instructor, and exploring further educational opportunities.
• Prioritize excellent customer service, ensuring accessibility to care while actively participating in initiatives that reduce barriers to effective rehabilitation.

Patient Population / Client Focus

The primary focus of this role will be on patients requiring rehabilitation in their homes, including individuals recovering from surgery, managing chronic conditions, or recovering from injuries. Your expertise will be crucial in tailoring rehabilitation plans that help these patients regain mobility, strength, and confidence in their daily activities.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required to ensure you bring top-level expertise to our patient care.
• Must hold a valid state license to practice Physical Therapy in New Jersey.
• American Heart Association's BLS CPR certification is mandatory to ensure the safety of our patients during care.
• Strong communication and interpersonal skills are essential for collaborating with patients, families, and interdisciplinary teams.

Preferred Experience

While a minimum of two years of relevant experience is required, we also value individuals who are committed to professional development and actively seek educational opportunities to enhance their clinical skills.
