Key Responsibilities

• Conduct comprehensive evaluations of patients, including the review of medical histories and performance of specific assessments, to develop tailored treatment plans that are documented thoroughly.
• Effectively communicate evaluation findings and treatment recommendations to patients and their families, involving them in goal-setting to foster engagement and accountability in their recovery.
• Implement treatment protocols with precision, adjusting approaches based on individual patient presentations and progress, while adhering to best practices within an evolving healthcare landscape.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure well-rounded care, promptly recognizing changes in patient conditions that may require revisions in the treatment plan.
• Create discharge plans and follow-up care strategies, maintaining clear and timely documentation that reflects the patient's rehabilitation journey, including initial evaluations, progress notes, and discharge summaries.
• Manage time efficiently to meet productivity expectations, maintain an organized schedule, and participate actively in departmental meetings and community outreach initiatives.
• Contribute to the achievement of departmental goals, ensuring compliance with all regulatory standards and cultivating a workplace culture focused on quality improvement.
• Engage in personal and professional growth through continuing education opportunities, potentially serving as a clinical instructor to students.
• Deliver exceptional customer service, adapting flexibly to the needs of patients while removing barriers to access healthcare services.

Patient Population / Client Focus

The patients you will serve at Swift Health range from post-operative individuals seeking recovery to those with chronic conditions requiring ongoing management. This role allows you to develop meaningful relationships with patients, understanding their unique challenges, aspirations, and home environments, leading to a more personalized approach to rehabilitation.

Qualifications

• Bachelor's degree in Physical Therapy; a Doctorate degree in Physical Therapy (DPT) is preferred.
• A valid physical therapy license in the state of New Jersey is required.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• A commitment to continuous professional education and improvement.

Preferred Experience

• A minimum of 2 years of experience as a practicing Physical Therapist is required.
• Experience in home healthcare or outpatient physical therapy settings is highly desirable, as it aligns closely with our patient care model.
• Strong written and verbal communication skills for effective patient interactions and documentation.

Compensation and Benefits

Swift Health offers a competitive salary range of USD 100,000 to 120,000 annually, complemented by a robust benefits package.
