Work Environment

This full-time position is located in Woodbridge and requires onsite presence to meet patients in their home environments. Flexibility is critical, as weekly schedules consist of 40 hours along with variable weekends. The home care setting provides a unique opportunity to forge meaningful relationships with patients, engage in community outreach, and directly influence positive health outcomes in familiar surroundings that promote recovery.

Key Responsibilities

• Conduct thorough examinations and evaluations of patients to assess physical conditions, functional limitations, and individual rehabilitation needs.
• Design and implement effective treatment plans tailored to each patient's unique circumstances, ensuring adherence to professional standards and legal regulations.
• Maintain accurate documentation of evaluations, treatment sessions, and discharge plans, including tracking patient progress and incorporating feedback.
• Collaborate with interdisciplinary teams, physicians, and referral sources to deliver comprehensive patient care throughout their recovery journey.
• Engage in community outreach initiatives and quality improvement (CQI) projects, enhancing service visibility and organizational effectiveness.
• Supervise and mentor assigned support staff, fostering a culture of learning and professional growth within the department.
• Attend regular staff meetings, training sessions, and continuing education programs to maintain competency and stay current with industry practices.

Patient Population / Client Focus

This role primarily involves working with a diverse clientele, including patients recovering from orthopedic surgeries, sports injuries, or those suffering from chronic pain conditions. Physical Therapists will assess functional abilities, provide hands-on therapy, and ensure that patients and their families are engaged in the therapeutic process through education and empathetic communication.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• A valid State License to practice Physical Therapy in New Jersey is essential.
• Basic Life Support (BLS) certification must be current.
• Excellent written and verbal communication skills are necessary for effective patient interactions and documentation.
• Proficiency in word processing and familiarity with electronic health records (EHR) is required for documentation and communication.

Preferred Experience

While a minimum of 2 years of physical therapy experience is required, candidates with a background in home care settings, orthopedic rehabilitation, or community health initiatives are preferred. Experience with patient assessments and developing individualized care plans will enhance your suitability for this role. Involvement in leadership or mentorship activities is a plus.
