Access Control Physical Therapy & Wellness is actively looking to fill the role of a Licensed Physical Therapist for our home care program dedicated to providing exceptional rehabilitation services. Work Environment

This full-time position is primarily onsite, allowing therapists to engage directly with patients in the comfort of their homes. Our home care setting ensures that you can observe patients in their daily living environments, allowing for tailored and relevant treatment. Operating hours are from 8:00 AM to 5:00 PM, Monday through Friday, facilitating an excellent work-life balance. Key Responsibilities

• Maintain a comprehensive understanding of state laws and practice acts governing physical therapy, ensuring compliance in all aspects of your practice.
• Assess, examine, and evaluate patients to understand their functional limitations and disabilities, developing tailored treatment plans that promote recovery and enhancement of quality of life.
• Conduct in-depth evaluations through clinical assessments and standardized tests, documenting your findings meticulously and creating actionable recommendations.
• Implement treatment protocols based on evaluation results, optimizing your approach to coverage of diverse cases while adapting to the evolving healthcare landscape.
• Coordinate with other healthcare providers, recognizing when multidisciplinary collaboration is necessary to modify treatment plans based on patient progression.
• Formulate and execute effective discharge plans to facilitate smooth transitions from therapy to independence, ensuring patients receive necessary follow-up care.
• Maintain accurate records of patient encounters, progress summaries, and discharge evaluations to provide transparent care documentation.
• Comply with ongoing professional training requirements and all pertinent regulations to maintain the highest standard of care.
• Participate in team meetings and contribute to enhancing access initiatives within our organization.

Our focus primarily lies within the geriatric population, as well as individuals transitioning back to their daily activities after orthopedic surgeries or severe injuries. Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited institution is required.
• Must hold a current state license as a Doctor of Physical Therapy (DPT).
• A minimum of two years of experience in outpatient orthopedics is necessary for consideration.
• Current CPR certification is also required to ensure safety in clinical practices.

Preferred Experience

• Experience in home care settings is highly desired, with the ability to adapt treatment strategies to unique patient environments.
• Familiarity with electronic documentation and telehealth processes is advantageous for streamlined operations.

Compensation and Benefits

The position offers a competitive salary ranging from USD 100,000 to 120,000 annually, along with a comprehensive benefits package that includes medical, dental, and vision insurance.
