Desert Blossom Orthopedic and Sports is currently seeking a dedicated and compassionate Licensed Physical Therapist to join our team in Keyport, New Jersey. Work Environment

This position is fully onsite, as you will be working directly in the homes of our patients throughout the Keyport area. Our physical therapists are equipped with the necessary tools to provide care in diverse settings, allowing you to perform evaluations, therapeutic exercises, and home modifications in real-life scenarios. Collaboration with a multidisciplinary team, including case managers and physicians, ensures a holistic approach to patient care.

Key Responsibilities

• Conduct comprehensive evaluations of clients' functional abilities using diagnostic exercises and assessments to establish treatment plans tailored to their individual goals and capabilities.
• Direct and supervise the implementation of physical therapy programs, ensuring quality and adherence to care plans.
• Evaluate and measure clients' progress, making necessary adjustments to the treatment protocol based on observed changes in condition.
• Develop and recommend modifications for home environments, aimed at enhancing the client's independence and safety in daily activities.
• Administer a variety of therapeutic exercises and employ manual therapy techniques to alleviate pain and enhance mobility.
• Guide clients and families on the effective use of prescribed physical therapy devices and the overall care plan.
• Keep detailed clinical records, including initial evaluations, progress notes, and discharge summaries, in line with compliance regulations like OASIS.
• Participate in interdisciplinary team meetings to coordinate care plans and share insights regarding patient progress.
• Train and supervise Physical Therapy Assistants (PTAs) in accordance with state regulations and ensure proper implementation of therapeutic techniques.
• Stay accessible to PTAs to provide guidance during treatment sessions and assess the effectiveness of therapies applied in clients' homes.

Patient Population / Client Focus

In this role, you will primarily work with patients recovering from orthopedic surgeries, sports-related injuries, and individuals who require support following acute medical conditions. Our focus is on delivering individual plans that accommodate each client's specific circumstances, promoting their rehabilitation while respecting their preferences and surroundings. You will engage with clients from diverse backgrounds, each with unique needs and challenges, making your role vital in their healing process.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active Physical Therapist license in New Jersey without any sanctions or board proceedings.
• Proficient in a range of manual therapy techniques and the ability to develop customized exercise programs based on individual assessments.
• Familiarity with electronic medical record (EMR) systems, particularly Web PT, for effective documentation and billing.
• Strong capability to stay current with evidence-based practices in physical therapy.

Preferred Experience

• Experience in home healthcare settings is highly desirable, enabling you to adapt care plans to clients' living environments.
• Background in working collaboratively within a multidisciplinary team to enhance patient management.
• Knowledge of regulatory and compliance requirements associated with home health services to ensure quality and legal adherence.

Compensation and Benefits

We offer a competitive salary range of $100,000 to $120,000 per year, commensurate with experience, along with a comprehensive benefits package.
